What Are Cochlear Implants And How Do They Work?

Cochlear implants are medical devices that provide a sense of sound to those with profound deafness. Unlike hearing aids that amplify sounds to be detected by damaged ears, cochlear implants bypass damaged portions of your auditory system to stimulate the auditory nerve directly and allow hearing.

Here’s how cochlear implants work:

  • Microphone: The microphone is worn outside the ear. It helps in receiving sounds from the environment and converting them into digital signals.
  • Speech processor: Worn behind the ear (just like a traditional hearing aid), it selects and arranges the sounds received by the microphone, converts them into digital signals, and sends them to the transmitter. 
  • Transmitter: The transmitter is also worn externally; it processes the signals and transmits the same to the receiver or stimulator via a magnetic link through your skin.
  • Receiver/Stimulator: This component is placed under the skin. It receives signals from the transmitter, converts them into electric impulses, and is connected to an array of electrodes surgically inserted into the cochlea. 
  • Electrode array: An electrode array is a thin wire with electrodes along the length. When electric impulses are sent through the array, the auditory nerve fibers are stimulated in the cochlea. The electrical impulse patterns represent sound signals and mimic the function of the damaged hair cells in the cochlea.   
  • Brain’s interpretation: Finally, the brain receives the auditory nerves and recognizes them as sounds. Practice and rehabilitation are required to interpret the signals into meaningful sounds and speech.

Note that cochlear implants will not restore normal hearing. However, they’ll help the deaf represent sounds and understand speech. Their effectiveness depends on factors including the level of deafness, the age of implantation, and your auditory nerve’s health.

When Is A Cochlear Implant Surgery Recommended?

Cochlear implant surgery is required for individuals with hearing loss in both ears and little to benefit from conventional hearing aids. The surgery is primarily recommended for the following conditions:

  • Severe to profound hearing loss: Individuals with over 70% loss of cochlear hair cell function, inability to hear most sounds at any volume, and significantly poor understanding of speech might be candidates. 
  • Limited benefits from hearing aids: Traditional hearing aids are tried on the patient before opting for the surgery.
  • Post-lingual hearing loss: Cochlear implants specifically benefit those adults and children who lost hearing after acquiring language. However, pre-lingual deafness in children may also be treated with this surgery, ideally at a young age to support language development.
  • Age and health considerations: Suitable candidates include infants as young as 6-12 months to elderly individuals. They should be in overall good health.
  • Motivation and realistic expectations: The individual should be committed to rehabilitation post-surgery, including auditory training and speech therapy. Carrying realistic expectations by the patient and his/her family is also important.
  • Support system: A support system including family and friends with educational and rehabilitation services should be there, especially in the case of children. 
  • Bilateral implantation: Sometimes, implantation in both ears is recommended to improve one’s ability to localize sound and also understand speech in noisy environments.
