Anatomy Of The Breast

The anatomy of the breast is a complex one, that consists of multiple structures such as glandular tissue, connective tissue, fat, lymph nodes, blood vessels, and nerves. Here’s a detailed overview:

  • Glandular tissue: The breast is primarily composed of glandular tissue. This tissue has lobes and lobules. There are 15 to 20 lobes in each breast, with lobules located inside the lobes. The lobes radiate around the nipple and the lobules contain the milk-producing glands called alveoli. Milk is carried from the alveoli to the nipple via ducts.  
  • Fatty tissue: Fatty tissue surrounds the glandular tissue and forms a major part of the breast. Its proportion can vary from individual to individual, depending on and changing with the person’s weight, age, and hormonal changes.   
  • Connective tissue: The connective tissue provides support and structure to the breasts. It has ligaments known as Cooper’s ligaments, that hold the breast tissue onto the wall of the chest. 
  • Nipple and areola: The nipple refers to the projection right in the middle of the breast and the areola is the dark area surrounding it. 
  • Blood supply: There are blood vessels that circulate throughout the breasts, chest, and body.
  • Lymphatic system: The lymphatic system in the breasts is responsible for draining lymph (fluid) and for immune defense. Lymph vessels connect to the lymph nodes found under the armpits, in the chest, and in other places. 
  • Sensory nerves: There are hundreds of nerve endings in the nipples, which make them extremely sensitive to touch.

Here’s a detailed overview of what you can expect before, during, and after a breast reduction surgery.

Before Surgery

The pre-operative phase is about consultation and evaluation:

  • Initial consultation: Initially, you speak with a doctor to discuss your surgery goals and your medical history along with details of any medications you are on.
  • Physical examination: The doctor examines your breasts, takes required measurements, and may click photos for medical records. This involves evaluating your breast size and shape, skin quality, and the placement of the nipples and areolas. 
  • Technique discussion: The technique of breast reduction is discussed and decided based on examination, possible risks and benefits, and expectations.
  • Preoperative instructions: You may be instructed to adjust your current medications if any and to stop smoking to reduce possible postoperative complications.
  • Mammogram: A baseline mammogram may also be recommended for some patients.

During Breast Reduction Surgery

The surgery is carried out in the following manner:

  • Anesthesia administration: You are administered general anesthesia to put you into temporary sleep for the duration of the surgery.
  • The surgery: The surgery can be 2-5 hours long. It involves making incisions, removing the extra fat, tissue, and skin, and reshaping the remains, based on the technique applied. There may also be a repositioning of the nipples and the areolas to give them a more natural and youthful height. 
  • Closure: Finally, sutures are used to close the incisions, and the breasts are then wrapped in gauze or a surgical bra. 

After Surgery

The immediate postoperative period after a breast reduction operation is as follows:

  • In the recovery room: Immediately after the completion of the surgery, you are shifted to a recovery room. Here, your vitals are monitored once you wake up from the effect of anesthesia.
  • Managing pain: Medications are given to help manage pain and discomfort, which are commonly possible during the immediate postoperative period.
  • Drains: Sometimes, drains are also placed to remove excess fluid. The drains are later removed, probably post a few days.
