Overview

Bipolar cautery vaporization, or “Button TURP'' is an advanced procedure for treating an enlarged prostate. TURP (Transurethral Resection of the Prostate) has been a gold standard treatment for low urinary tract symptoms (LUTS) due to benign (non-cancerous) prostatic hyperplasia (an abnormal increase of the cells). Bipolar cautery vaporization is performed using a special scope inserted through your urethra (a tube through which urine leaves the body) under general anesthesia (a numbing agent). This scope consists of a light source, a tiny camera to view the prostate tissues,  and a button-shaped device at the end that uses plasma energy to dissolve (vaporize) the tissues that often cause urinary obstruction in men.
 

What is Bipolar Cautery Vaporization?

An enlarged prostate is a common condition among aging men, which makes it harder for them to urinate and fully empty the bladder. This leads to frequent urination and urine incontinence (inability to control the urine flow that causes leakage). 

The severity of BPH can be classified into the following grades or stages:

  • Stage I- No significant urethral obstruction is present.
  • Stage II- There are bothersome symptoms like the frequent urge to urinate, etc., but there is no urethral obstruction.
  • Stage III- There is significant urethral obstruction.
  • Stage IV- Severe obstruction with symptoms like urinary retention, hematuria, and recurrent UTIs.

Transurethral resection of the prostate (TURP) is an effective treatment that helps treat an enlarged prostate, with bipolar cautery vaporization being the most advanced technique. It is used to treat stage III and IV BPH.

Bipolar cautery vaporization, or button TURP is a minimally invasive, incision-free procedure that utilizes a special scope, a resectoscope with a light source, and a tiny camera inserted through your urethra. The camera captures images of the surgical site and projects them on a large scale to guide the surgeon in performing the surgery. A button-shaped device at the end of the resectoscope uses low-temperature plasma energy instead of heat or electrical energy to vaporize (dissolve) the enlarged, hyperplastic prostate tissues. 

When is Bipolar Cautery Vaporization Recommended?

Bipolar cautery vaporization is most often recommended to reduce symptoms caused by benign prostatic hyperplasia (BPH). These include:

  • Frequent urination, especially at night.
  • An uncontrollable urge to urinate.
  • Slow and prolonged urination.
  • Inability to completely empty your bladder.
  • Recurrent urinary tract infections.

  • Your urologist will first record your medical history for the following:

    • To determine the type of medications being taken, including blood thinners, anti-inflammatory and antidepressant drugs, or other herbal supplements.
    • To identify any allergy to anesthesia, latex, or other medications.
    • To evaluate any history of previous surgeries or injuries.
  • You will later undergo a few tests to confirm the diagnosis, which include:
    • Blood tests- To check for prostate-specific antigen (PSA).
    • Urinalysis and culture test- To detect the presence of any urinary tract infection.
    • Pressure flow study- To evaluate the pressure inside the bladder and the urine flow rate.
    • MRI (Magnetic Resonance Imaging) and CT (Computed Tomography) scan- To check for the size of your hyperplastic prostate.
  • Your surgeon will give the following instructions once you are considered suitable for the bipolar cautery vaporization surgery in Rewari:
    • You must stop your medications (blood thinner, etc.) at least two weeks before the procedure to prevent bleeding during and after surgery.
    • You should not smoke or consume alcohol for a period of 2 to 3 weeks before the surgery, as it may delay the healing.
    • You should fast 6 to 8 hours before the procedure since it will be done under general anesthesia (GA), so the food should not aspirate into the lungs.
    • You will be made to sign a consent form that gives permission for the surgeon to perform the procedure. 

How Does The Procedure Work?

Bipolar cautery vaporization is an inpatient procedure that may require you to stay in the hospital for 1 to 3 days for the initial recovery. The procedure involves the following:

  • On the day of your surgery, your nurse will prep you by shaving the area around your urethra and disinfecting it with an antiseptic solution. 
  • Your surgeon will administer general anesthesia through an IV (intravenous) infusion and put you to sleep.
  • A resectoscope with a button-shaped device or electrode will be inserted by your surgeon through the urethra. 
  • The images captured through the resectoscope camera will be projected on a large screen to guide the surgeon in performing the surgery.
  • The surgeon will guide the resectoscope electrode near the prostate to dissolve or vaporize the hyperplastic prostate tissues. 
  • Your surgeon will then slowly retract the resectoscope and insert a tube known as a catheter that will be kept in place for around one week following the surgery.
  • The entire procedure will take around 60 to 90 minutes, depending on the extent of the hyperplastic prostate tissue that needs to be removed. 

What to Expect After a Bipolar Cautery Vaporization?

You are expected to spend another 1 to 2 hours in the recovery room, where you will be observed by monitoring your vitals (blood pressure, sugar levels, pulse, heart rate, oxygen levels, and urine output).

  • Once stabilized, you will be shifted to your ward for further recovery.
  • You may experience mild pain, soreness, and discomfort once the effect of your anesthesia wears off. This usually resolves gradually over a period of time. However, your surgeon will prescribe painkillers to ease the pain and discomfort.
  • Your urine output will be monitored to check for the amount of urine and the drainage post-surgery through the urine collected in the catheter bag.
  • Once you return home, it is common to experience weakness for one-week post-surgery.
  • You must stay off work, avoid lifting heavy objects, strenuous exercises, driving, and having sex for at least a few weeks.
  • You may experience difficulty urinating or even notice some traces of blood in your urine during your recovery period, which should improve as you heal.
  • Complete recovery may take around 4 to 6 weeks.
