This includes employees from various ministries, departments, and organizations under the Central Government.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Pensioners:<\/b><span style=\"font-weight: 400;\"> Central Government pensioners receiving a pension from the Central Civil Estimates are eligible for CGHS. In case you\u2019re new or have lost the password, generate\/reset a new one.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Check the beneficiary details, go to the next page, select an option from CGHS dispensary, specialty, and doctor, and then click on \u2018Proceed\u2019 to go to the next page.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Book an appointment on the displayed calendar and select proceed. You will receive a confirmation message. Confirm the appointment and you\u2019ll again receive confirmation on your registered mobile number.<\/span><\/li>\n<\/ul>\n<h2><b>CGHS Card<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">The benefits of the CGHS scheme can be availed with the help of the CGHS card. It is a white-colored card with your (beneficiary) photo on it. The card also contains the beneficiary number and a strip that can be one of the following in color:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Yellow for beneficiaries of autonomous bodies\/journalists<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Blue for serving government employees<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Green for pensioners, freedom fighters, ex-MP, etc.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Red for members of the Parliament of India<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">This card is valid till retirement. It can be renewed, but the scheme cannot be transferred.<\/span><\/p>\n<h3><b>CGHS Card: Terms and Conditions<\/b><\/h3>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The misuse of the card results in penalties and disciplinary actions.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Upon the card\u2019s expiry, it should be surrendered to the concerned department.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">For its renewal, you will need to fill out the form with other necessary details.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Only the eligible person\/cardholder can enjoy the benefits.<\/span><\/li>\n<\/ul>\n<h3><b>How to Apply for the CGHS Card?<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">If you are eligible for the scheme, you can apply for the card online as well as offline.<\/span><\/p>\n<p><b>Online:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Visit the earlier-mentioned website, fill out the online application form, and attach your photos in the given designated areas<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Attach the needed documents to the form<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Submit the same with documents online<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Download the form and then take out its printout<\/span><\/li>\n<\/ul>\n<p><b>Offline:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">To submit the application form offline, you need to ensure that the form is endorsed by the HOD (head of the department) of serving employees. The form is to be submitted to Delhi\u2019s Additional Director headquarters office. The form, along with all documents, can be sent to the Additional Director\u2019s office in other cities.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">In case you\u2019re a serving employee, submit the application with a bank draft and enclosures to the CGHS from the employee\u2019s office.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">One may further note that the CGHS card has some validity and does not come completely free of cost. A central government employee can avail of its benefits post-retirement by making a contribution a year before completion. The card is valid in all impaneled govt\/private hospitals (hospitals that come under the CGHS scheme regions) for hospitalization, diagnosis, and medications.<\/span><\/p>\n<p><strong>Coming to the card\u2019s transfer, here are some points to note:<\/strong><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If the cardholder is a central government employee who is being transferred to a non-CGHS region, the card will be used for 6 months by the beneficiary\u2019s family if a contribution has been made beforehand.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If the transfer is done in the same city, the requisite changes are made by the concerned department in the employee database.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Also, a transfer permit is needed to receive treatment in another CGHS city for life-saving medications.<\/span><\/li>\n<\/ul>\n<h2><b>Documents Required for the Scheme<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Documents to be attached to the application form are:<\/span><\/p>\n<p><strong>For pensioners:<\/strong><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bank demand draft<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">(Pension Payment Order) PPO\/Provisional PPO\/Last pay certificate (LPC) copies<\/span><\/li>\n<\/ul>\n<p><strong>For serving government employees:<\/strong><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Address proof<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Dependent\u2019s residential proof<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Dependent\u2019s age-proof<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">A certificate of disablement from a valid authority, in case the dependent is physically disabled<\/span><\/li>\n<\/ul>\n<h3><b>Conclusion<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">The CGHS or Central Government Health Scheme has been offering several medical benefits to the beneficiaries and their families.
