Earlier this scheme was meant for the people below the poverty line. However, later the benefits of the scheme were extended to lower-middle class families also.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">\u00a0<\/span><b>Features of the Mukhyamantri Amrutum Yojana\u00a0\u00a0<\/b><\/h2>\n<p><strong>Here are some of the salient features of this scheme:\u00a0<\/strong><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Those eligible to receive the benefits of this scheme can get medical and surgical treatment at the empanelled hospitals. You\u2019ll be issued a card (Vatsalya Card) which will contain the list of such hospitals.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The beneficiaries can just show their card at the empanelled hospital and do not need to pay anything for the treatment. This means it\u2019s a cashless scheme.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The scheme, also known by the name Mukhyamantri Amrutum Vatsalya Yojana, covers your transportation charges, medical consultation, hospitalization, treatment and surgery.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">It is the duty of the hospitals to arrange all the medicines and tests required by the patients.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">There is no age bar; the names of the family members should be enrolled in one ration card.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">When required, one can inquire about anything related to the hospital by calling the toll-free number 1800 233 1022.<\/span><\/li>\n<\/ul>\n<h2><span style=\"font-weight: 400;\">\u00a0<\/span><b>Benefits of the Scheme<\/b><\/h2>\n<p><strong>\u00a0Some of the important benefits receivable by the people of Gujarat under this scheme are:<\/strong><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The scheme covers 1,763 medical procedures.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">It lets you claim cashless treatment of up to Rs. 5 lakhs.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">For every visit to the empanelled hospital, the beneficiaries are given Rs. 300 as the transport charges.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Families with an annual income of up to Rs. 4,00,000 per annum can also avail of these benefits along with the families which come below the poverty line.<\/span><\/li>\n<\/ul>\n<h2><span style=\"font-weight: 400;\">\u00a0<\/span><b>Diseases and Treatments Covered Under the Scheme<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Here is a list of some of the diseases covered under this scheme, which benefits all MA Amrutam cardholders:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Burns, polytrauma, renal disease<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Liver, pancreas, kidney transplant, etc.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Joint replacement<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">Neonatal diseases<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Cardiovascular disease<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Neurological disease and surgery<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Hip and knee replacement<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Cancer (malignancies)<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">There are some exclusions as well. The scheme does not cover some non-emergency procedures and some of the cosmetic treatments such as plastic surgery meant for better appearance.<\/span><\/p>\n<h2><b>Eligibility Criteria<\/b><\/h2>\n<p><strong>The following are the eligibility conditions:<\/strong><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The families should have an annual income of up to Rs. 4 lakhs.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Accredited reporters.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You need to be a U-win cardholder.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">All ASHA or Accredited Social Health Activist workers are also eligible, both urban and rural.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Employees of classes 3 and 4, appointed by the state government and having a fixed pay.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Elderly people coming from households with an annual income of Rs. 6 lakhs.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Beneficiaries whose information is there on the district BPL list created by the rural and urban development of the state government; and who come from BPL (below the poverty line) families.<\/span><\/li>\n<\/ul>\n<h3><b>How to Enroll in the Mukhyamantri Amrutum Yojana<\/b><\/h3>\n<p><strong>Here is the enrolment process:<\/strong><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You need to visit the Civic Center kiosk at a Taluka or visit the mobile kiosk.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Enrolling will require your biometric information such as iris scan and fingerprints, along with your family members\u2019 photographs.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">At the Civil Center or the kiosk office, you first need to fill out the required Mukhyamatri Amrutum card application in Gujarati as well as proper family details.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Your documents will be verified, after which you will be given a related pamphlet and the MA card.<\/span><\/li>\n<\/ul>\n<p><strong>The documents required for enrolling at the Nodal agency include<\/strong><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Proof of identity (PAN card, Adhaar card, Driving license or Voter ID card)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">BPL certificate<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Income certificate<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Address proof (ration card)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Mobile number<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Passport size photograph<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Moreover, to complete the enrolment process, you will need to visit the Nodal Agency. You will get the MA Vatsalya card application form pdf or a physical form that you\u2019ll need to fill out and submit.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">After the process, you can check your status online and offline. To check the status of your card, you can visit the kiosk again, or visit the MA Gujarat website and use your login details to check the status. The website will help you know a lot of details related to the scheme.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In case your MA Vatsalya card is lost, you can get a new one at the Taluk kiosk. Submitting the MA card document will let you avail of the coverage in times of medical emergency.<\/span><\/p>\n<h3><b>Claiming the Benefits of the Scheme<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Once you are successfully enrolled, you can visit your nearest network\/empanelled hospital and get the diagnosis done. After submitting your MA health card and other necessary documents, the medical institution will seek pre-authorisation from the insurer. Once it is approved by the insurer, you can avail the treatment. Post-treatment, the hospital will send all documents to the insurer for settlement.\u00a0\u00a0<\/span><\/p>\n<h3><b>A Few Important Points to Note<\/b><\/h3>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Your MA card is valid for 3 years from the date of issue.
