Overview

‘Endo’ refers to the usage of scope (a thin long tube flexible tube) during the procedure. ‘Pyelo’ refers to the center part of the kidney or kidney pelvis where the uterus is starting. ‘Tomy’ means to cut.

Endopyelotomy definition - it is a minimally invasive surgical procedure used to treat a blockage in the renal pelvis, the area where the kidney collects urine before it is transported to the bladder. The endopyelotomy procedure involves using an endoscope, which is a flexible tube with a camera and a light, to access the blocked area and make a small incision to remove the obstruction.

What Is Endopyelotomy?

Endopyelotomy refers to the surgical procedure used to treat pelviureteric junction (PUJ) obstruction. This is a condition in which the renal pelvis of the kidney narrows down. The renal pelvis is nothing but a funnel-shaped structure that collects urine before it is carried by the ureters to the bladder.

The obstruction can cause symptoms such as pain, nausea/vomiting, kidney stones, and urinary infections. The normal function of the kidney could also be affected in some cases. This procedure aims at widening the narrowed area through the removal of the tissue causing the obstruction.

Types Of Endopyelotomy

There are two types of endopyelotomy:

  1. The antegrade approach: Here, the kidney is accessed via a small incision in the skin. This is done through a percutaneous nephrostomy tube from the outside (puncturing the kidney).
  2. The retrograde approach:  Here, the surgeon passes a tiny telescope through the urethra up to the kidney.

For additional info, one may know that there exist acucise endopyelotomy systems. Acucise endopyelotomy is a minimally invasive technique for the treatment of UPJ or ureteropelvic junction obstruction.

Before The Procedure

  • You can discuss your medical history with your doctor along with current medications and other ailments if any.
  • You must also inform him/her about an implanted foreign body such as a stent, pacemaker, joint replacement, etc.
  • You will be asked to quit smoking for a brief period before and after the procedure as it can interfere with healing.
  • You need to do a bowel prep right on the day before surgery. Your doctor will suggest diet changes in the days before the procedure.
  • Make sure to arrange help on the day of the procedure and during your recovery period.

What Happens After The Procedure?

  • The stent is removed under local anesthesia after about 4-6 weeks, which is the endopyelotomy stent placement duration.
  • Then, further imaging (scans or X-rays) are arranged to check the success of the operation and to ensure that the kidney is draining well.
  • For the first 2-3 days, drink more fluid than you normally do. This reduces the risk of urine blockage due to blood clots and infection.
  • Usually, you can return to work within 1-2 weeks after the procedure. Recovery can be longer in the case of a physically demanding job.
  • You can have a high-fiber diet and drink plenty of fluids to improve digestion and prevent the risk of constipation during your recovery.
  • Make sure to avoid heavy lifting and other strenuous activities for 3-4 weeks.
  • Have all medications as prescribed by your doctor and follow all the post-operative instructions for a faster recovery.
  • Attend all follow-up visits as instructed.

When To Contact The Doctor?

Contact your doctor in the following cases:

  • High Fever
  • Pain in the region of the affected kidney
  • Severe pain/burning while urinating
  • Fresh bleeding or blood clots
  • Inability to urinate or low amount of urine

Risks or Complications Associated With Endopyelotomy

While endopyelotomy is generally a safe and effective procedure, like all medical procedures, it carries some risks. Here are some potential complications associated with endopyelotomy in Mysuru:

  • Bleeding: Endopyelotomy can cause bleeding, either during the procedure or afterward. In some cases, bleeding may require further medical attention.
  • Infection: Any surgical procedure carries a risk of infection. Your doctor will take steps to minimize this risk, such as giving you antibiotics before and after the procedure.
  • Damage to surrounding organs: Endopyelotomy involves making a small incision in the renal pelvis, which is close to other organs such as the ureter and bladder. There is a small risk of damaging these organs during the procedure.
  • Recurrence of the blockage: While endopyelotomy is effective at treating blockages in the renal pelvis, there is a small chance that the blockage may return in the future.

Following the post-operative instructions thoroughly will avoid the risk of complications and help you have a faster recovery.

What Happens During an Endopyelotomy?

The general steps that you can expect on the day of an Endopyelotomy procedure are as follows:

  • Anesthesia: Endopyelotomy is usually performed under general anesthesia, which means you will be asleep during the procedure.
  • Insertion of the endoscope: Once you are asleep, your doctor will insert a small, flexible tube with a camera and light called an endoscope into your urethra and up into your bladder.
  • Making the endopyelotomy incision: Using a small surgical instrument that is inserted through the endoscope, your doctor will make a small incision in the blocked area of your renal pelvis (endopyelotomy incision location).
  • Placing a stent: Once the blockage is removed, endopyelotomy stent placement is done. An endopyelotomy stent is left in place for a few weeks. This will allow the urine to flow freely from your kidney into your bladder as you recover.

Once the procedure is completed, you will be taken to a recovery room, where you will be monitored closely as you wake up from the anesthesia.
